Appointment enquiries
Our form requests your first and last names, email address and phone number, with an optional booking message. With your consent, these details are used to respond to your enquiry, discuss appointment availability and email your request to the clinic’s monitored inbox.
Keep health information out of this form
Do not send diagnoses, medical history, reports, health-card numbers or other health information through this website or ordinary email. Clinical information should be shared through the clinic’s appropriate intake process.
Storage and access
Contact details and booking messages are encrypted in the website’s enquiry storage, with access through the protected clinic dashboard. Enquiries are designed to be deleted after 30 days. The clinic may retain a separate record where needed to respond or provide care. Copies in the clinic inbox and hosting backups are managed separately under the clinic’s retention practices. Website deletion does not automatically delete those copies.
Cookies and logs
Essential session cookies protect forms and administrator access. This version contains no advertising trackers or third-party analytics. Hosting providers may keep security and operational logs including IP addresses. Short-lived hashed identifiers are used to limit repeated requests.

External websites have their own privacy practices. Ordinary email is not a secure clinical intake channel.
